If you, or someone you know, would like to join the register of disabled children and young people in Manchester you can contact us in any of the ways listed opposite to refer yourself. Alternatively, please print out & complete our leaflet and registration form and send it back to MAP at the freepost address opposite (you will not need a stamp).
